In addition to the normal documents sent to the insurance company to enable a claim to be paid, (such as claimant statement, original policies, lost policy statement, death certificate, funeral bill and premium receipt book), the following forms should also be sent to us.

The Irrevocable Assignment form is to be signed by the beneficiary and  the Funeral Director and notarized. The Notary seal must be on the assignment.

The Standard claim form is to be completed and signed by the beneficiary if you do not have the insurance company claim form. We have most insurance company claim forms available, so ask and we can fax them to you. Also most companies are now requiring a photocopy of the beneficiaries drivers licence.

Itemized Funeral Bill, required by most insurance companies.

Before we can fund on any claims we must have either the Death Certificate, a photo copy of the Death Certificate, an Obituary and Program or a copy of the burial permit.

All signatures must be in longhand and all marks must be properly witnessed.
